Apply For A Medicaid Identification Number







Medicaid is a federally- and state-funded, state-operated health program for low-income individuals 65 and over or disabled, pregnant women and children. Medical providers must obtain a National Provider Identifier (NPI) number to bill Medicaid for services. You must have all necessary state and federal licenses to practice in your area prior to obtaining an NPI number.


Instructions


1. Gather information necessary for the application paperwork. This information includes your tax identification number--typically your Social Security number or federal employer identifier number--physical address of your office, telephone number and your full name or name of your business.


2. Contact 1-800-MEDICARE (1-800-633-4227) to obtain an application for an NPI number for your area of practice. Fill in the blanks with the information gathered in the previous step.


3. Complete a fingerprint card if required for your area of practice and submit it to the appropriate regional office. If you are unsure of which regional office to send the card to, you may obtain this information by contacting 1-800-MEDICARE.
