Family Health Plus is a NY State program that offers free or low cost health insurance to qualified individuals. It is meant for people who don't qualify for Medicaid, CHP or Medicare. Instructions
1. Contact the Department of Health to verify that you are not eligible for Medicaid, CHP or Medicare. This is necessary because if you can use those programs then you cannot sign up for Family Health Plus. You can contact them through their website, listed in the resource section.
2. Contact one of the plans. They can screen you for eligibility and arrange for you to meet with a representative. They will work with you to set up a time that is right for you. A list of State approved Family Health Plus companies is available below.
3. Ask the representative what paperwork you need. The documents that you need to sign up for Family Health Plus may vary from person to person, according to your financial situation, living arrangements and other factors.
4. Gather the required documentation and make photocopies of it before your meeting. Make sure each copy is on a separate piece of paper. Retain the originals to show the Department of Health and/or your health plan if necessary.
5. Go to the meeting. An application should be provided for you, and the agent can aid you in filling in the correct information.
6. Pick a doctor. The insurance company shall provide you with a list of doctors that accept Family Health Plus. Select the physician(s) of your choice from the list. You may always change your primary care physician at a later time if you so desire.
7. Wait for the company to send you any needed identification cards or information packets that describe your benefits. Once you receive these, make your first doctor visit using your Family Health Plus insurance.
